Senior Occupational Therapist - Golden Valley
Job Summary
Occupational Therapist – 12‑month fixed term contract, 37.5 hrs per week.
You have an opportunity to join our Neighbourhood Teams to provide a multi‑disciplinary approach alongside physiotherapists, therapy support workers, community matrons & district nurses. The Neighbourhood Teams aim to develop optimum levels of function and independence in the physical, emotional and social aspects of life and to prevent hospital or long‑term care admission and reduce inpatient length of stay.
Work autonomously within the community, liaising with other health and social care professionals, enabling people to facilitate discharge from the community hospital to return to their homes after illness or disability. Provide high quality OT assessments, direct rehabilitation, and manage patients with a range of conditions.
Key Responsibilities
* Provide a 2‑4‑week proactive response from referral.
* Deliver a rehabilitative approach to clients recovering from acute illness (including D2A referral pathways).
* Support planned hospital discharges with no urgent needs or care/long‑term package of care provider.
* Conduct short‑term assessment into care facilities.
* Assess and manage longer‑term conditions within an MDT.
* Develop longer‑term management plans, e.g. posture/seating and healthy lifestyles.
* Signpost/referring to wider community services (negotiating between ASC, neuro, etc).
* Support Hospital@Home (urgent response service).
* Triage and prioritise cases for effective caseload management, displaying organisation and discipline, working remotely and autonomously.
* Demonstrate decisive teamwork and maintain good rapport daily, remotely supervising team members.
Person Specification
Essential Qualifications
* Degree in Occupational Therapy.
* HCPC Registration.
* Post‑graduate specialist qualification or significant post‑graduate experience.
Essential Skills, Knowledge & Abilities
* Broad and specialist knowledge of relevant patient conditions and therapy interventions.
* Demonstration of core and specialist OT skills.
* Evidence of ongoing CPD in relevant area.
* Experience of multi‑disciplinary communication, assessments and record‑keeping.
* Computer skills including word processing and e‑mail.
Essential Experience
* Previous NHS or equivalent experience, e.g., inpatient, acute or community services.
* Experience of using or fitting adaptive equipment.
Contract & Compensation
* Pay scheme: Agenda for Change.
* Band: 6.
* Salary: £38,682 – £46,580 per annum pro‑rata.
* Contract: Fixed term, 12 months.
* Working pattern: Part‑time.
