About the role:
Want to be part of a team that really cares and empowers customers to live their best lives? As our Support Coordinator, you’ll provide the support needed for customers to develop skills for independent living in the community. Our service offers accommodation to customers aged 16–25 who are homeless or at risk of homelessness.

Typical day as a Support Coordinator:
1. Work with a team of Support Coordinators to deliver high-quality service meeting or exceeding KPIs and OFSTED standards.
2. Support and supervise customers to develop independence and achieve their goals.
3. Assist customers with mental health issues, housing support, complex needs, and managing challenging behavior.
4. Create and update support plans and risk assessments, helping with daily activities like cooking, shopping, budgeting, and maintaining tenancies.
5. Ensure properties are well maintained, clean, and homely to minimize voids.
6. Engage in regular chats and collaborate with colleagues.
What we’re looking for:
* Experience in creating person-centered support plans, risk assessments, and reviews.
* Experience in coordinating and assessing customer referrals.
* Ability to work independently, stay calm under pressure, and be resilient.
* Confidence to challenge behaviors misaligned with our values.
* Understanding of our customers' life stages and passion to advocate for them, with experience in housing or care support.

Job details:
* The service operates 24/7; you will work on a 4-week rolling rota, including weekends and bank holidays. Night shifts are not required.
* Occasional flexible hours may be needed to meet customer needs, and we aim to accommodate your work/life balance requests.
* Shifts may include lone working periods.
* Basic knowledge of Microsoft Word and Excel is essential.
* Ability to use technology for support plans, online learning, and collaboration.
* An Enhanced DBS check is required, which we will pay for.
