Job overview

An exciting and interesting opportunity has arisen for enthusiastic Community Recovery Support Workers to join the Community Enablement & Recovery Team (CERT) across the Black Country.

You’ll be working as part of a multi-disciplinary team of Recovery Practitioners, Occupational therapists, Psychology and Psychiatry, and work in partnership with local Authority, mental health services and third sector services, to meet the recovery needs of individuals accessing our service.

Our Service provides a recovery-based support and interventions in the community with individuals open to secondary care services and diagnosed with severe and enduring mental illness

You will need to have experience of working with individuals with mental health needs in the community

You will need to have excellent communication skills as you will be working with a wide range of internal and external partners and Service Users

You will be required to provide accurate written records and to feedback in a variety of settings

Main duties of the job

The post holder will be part of the implementation of care plans and community support interventions to service users experiencing severe mental illness within the Black Country.

The post holder will work under the supervision of Recovery Practitioners and will be managed by the Clinical Lead.

Detailed job description and main responsibilities

To work within the policies, protocols and clinical procedures of Black Country Healthcare NHS Foundation Trust and to promote competent working practice within an integrated multi-disciplinary team.

To liaise with Recovery Practitioner and carry out assigned duties as a member of the multi- disciplinary mental health team.

To be prepared to work on own initiative as necessary and report to Recovery Practitioner

Work flexibly from home and /or a community-based facility and provide service from the work base, via the telephone or within the service user’s place of residence.

The service will operate Monday to Friday 9-5 hours but is expected to advise service users and carers of access to services outside of these hours and liaise and refer on to other mental health care providers in cases where support outside of these hours is required.

Some travel will be required across the four areas within the trust

Promote equality for all patients and understand their health needs.

Demonstrate an awareness of service user and carer perspectives on the provision of treatment and continuing care.

Ensure accurate record keeping in accordance with Trust policy.

Participate regularly in clinical supervision sessions.

Liaise with representatives from associated voluntary and statutory organisations and facilitate good communication processes.

Attend in-service training and other study / refresher days to meet individual, statutory and service needs.

Participate in performance reviews.

Facilitate and participate in recreational and physical activities in the provision of health and social care to service users.

Person specification

Education

Essential criteria

1. Good general education to GCSE level or equivalent
2. Evidence of training / development
3. Good IT Skills

Experience

Essential criteria

4. Working within a Multi-disciplinary Community Care team
5. Working in partnership with other agencies
6. To be able to take responsibility for own work planning
7. Working with range of mental illnesses

Desirable criteria

8. Knowledge of and application of simple psychosocial interventions
9. Work in a social inclusion model
10. Planning and facilitating group work

Knowledge

Essential criteria

11. Demonstrate an understanding of Mental health symptoms
12. Demonstrate an understanding of Care Programme Approach/ Care Coordination
13. Demonstrate an understanding of Risk assessment and management
14. Demonstrate an understanding of Understanding of Recovery in mental health

Skills/Personal qualities

Essential criteria

15. Effective interpersonal, verbal and written communication skills
16. Ability to support service users with severe mental health needs to promote their own recovery
17. Ability to work independently and use own initiative
18. Demonstrate professionalism, i.e. time keeping, attitude, values
19. Ability to travel to numerous community settings around the Trust geographical area in a timely and effective manner
