Job Summary
We are looking for up to 2 experienced qualified practice nurses to join our team, based in Ilkley with potential to cross site work with our smaller practice in Grassington.

Main duties and responsibilities
* Support the Infection Prevention and Control (IPC) lead and work to IPC requirements by providing audits, training, support and evidence of compliance to the practice manager.
* Develop, implement and embed an effective care management programme for frail and housebound patients registered at the practice.
* Assess patient needs to ensure healthcare provision is appropriate and evidence-based.
* Develop, implement and embed health promotion and wellbeing programmes.
* Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
* Identify, manage and support patients at risk of developing long-term conditions to prevent adverse outcomes.
* Provide routine nursing care in line with clinical evidence, NICE and National Service Framework guidance as required.
* Provide wound care (including ulcers) and related procedures.
* Oversee chronic disease clinics and refer patients to secondary/specialist care as required.
* Maintain accurate clinical records in compliance with extant legislation and ensure SNOMED CT codes are used effectively by all nursing team members.
* Assist GPs with minor surgery when required and support the team in clinical emergencies.
* Implement vaccination programmes for adults and children and deliver opportunistic health promotion where appropriate.

Job Description and Responsibilities
To implement processes for the effective management of patients with long-term conditions, including care for elderly and housebound patients. As a senior member of the multidisciplinary team, ensure nursing services are delivered effectively to the entitled patient population. The Practice Nurse will oversee clinical areas such as infection prevention and control, health promotion, chronic disease management, well-woman/well-man clinics, and support the practice management team in reviewing and delivering clinical policy and procedures. The post holder will have access to clinical supervision and a designated supervisor for day-to-day support.

* Registered Nurse (NMC)
* Wound care, removal of sutures and staples

* Immunisations (routine, childhood and travel)

* Spirometry
* Level 2 Diabetes, or working towards
