What You’ll Do
* Deliver personalised care to individuals, families, and groups within the community, under the guidance of Specialist Public Health Nurses
* Work seamlessly within a multidisciplinary team to promote the Start Well programme and support the Family Hubs model
* Contribute to the innovative approach of the Integrated Children & Families Service across Oldham, with flexibility to work from various bases based on community needs

Proven post registration experience is required for this role.
Helping Children and Families Thrive: Your Key Contributions
As a dedicated member of our team, you’ll be instrumental in:
* Empowering Healthy Child Programmemes: Supporting the delivery of the Healthy Child Programme, a vital initiative that promotes the well‑being of children and families in our community
* Tailoring Health Support: Assessing the unique health needs of looked‑after children and young people, and providing targeted interventions to help them thrive
* Health Interventions with a Personal Touch: Delivering holistic health interventions to children and young people who need them most, ensuring they receive the care and support they deserve
* Conducting Health and Well‑being Reviews: Visiting homes, family hubs, and community settings to conduct comprehensive health and wellbeing reviews, helping families achieve their goals and work towards positive outcomes aligned with the Healthy Child Programme

To be eligible for this role you must have a current NMC PIN to practice as a Registered Nurse in the UK. An NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.
