Bridgend County Borough Council and Linc Cymru work in partnership to deliver a ‘Home for Life model’ for tenants and residents within an Extra Care community.
Ty Ynysawdre supports people to live as independently as possible taking into consideration their individual strengths and capabilities. Tenants at Ty Ynysawdre require a level of domiciliary support according to their assessed need which is provided by the in-house team. The service also provides 24 hr residential care for people living with dementia. Your role is to deliver person centred support to meet the identified needs of people who live in their own apartment or within 24 hr residential care suite.
You will work as part of a skilled social care team, delivering domiciliary or residential care to people living in Ty Ynysawdre. You will work closely with LINC colleagues, District Nurses, Social Workers, Therapists and other multiprofessionals across Health and Social care sector .
