Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please refer to the attached job description and person specification for full details of responsibilities • To work autonomously in the community setting under the direction of a registered practitioner within the integrated multi-professional ESD and neuro rehab team. • To work in a ward environment to support patients on the ESD pathway, working closely with the ward Multidisciplinary team on the ward and within the ESD/CNRS • To be responsible for assessing patient's physical and functional needs, interpreting, planning and implementing therapy interventions for patients with stroke and neurological problems. • To use clinical judgement to evaluate patient progress, including the recording of objective and subjective measurements. • To manage own caseload of patients who require ongoing management and support with complex needs and to plan and prioritise own workload. • To undertake duties delegated and supervised by a registered professional and following prescribed packages of care • To keep the patient at the centre of care, ensuring patient and carer participation in decision making.

Education / Qualifications
Essential criteria Educated to minimum GCSE level or equivalent grade A-C, including English RSA/OCR stage II - NVQ Level 3 in health and Social Care or equivalent experience
Desirable criteria GCSE or equivalent qualification in Math's, Biology and Food Technology 'A' level qualification or other post ideally in healthcare
Experience
Essential criteria Experience of working in rehabilitation in health/social care environment at band 3 level or equivalent Experience of working with people with neurological problems Experience / knowledge of rehab principles
Desirable criteria Has worked as a Rehabilitation Assistant in the past Experience of working with people with neurological problems
