To build on this success, we are recruiting an Advanced Clinical Practitioner (ANP) to join our team and ensure that our patients continueto receive the best possible care. Our practice has a strong culture oftraining and development, and we are a training practice for medical andnursing students. We are also committed to supporting the professionaldevelopment of our staff.

The ideal candidate will take responsibility as a seniormember of our dedicated nursing team, working closely with our management, GPs andadministrative teams to provide patient-centered care.

Main duties of the job

Clinically, your focus will be on delivering evidence-basedpractice to patients with long-term conditions, acute exacerbations, and minorinjuries/illnesses.

As an autonomous practitioner, you will be responsible forthe care delivered and will need to demonstrate critical thinking and clinicaldecision-making skills.

You will conduct both face-to-face and telephone triageappointments and work dynamically and collaboratively to meet the patients'daily needs.

In addition to these core responsibilities, you will beresponsible for implementing policies and procedures, as well as providing someleadership and mentoring for the nursing team.

We also encourage professional development and can offerinternal and external education to help you explore your specialist interest.

We are active in the local successful Primary Care Network (PCN) including having both Clinical Director and PCN Manager within our team.


Job responsibilities

The post holder will autonomously undertake assessment and treatment of adults (and children if qualified to do so) who present with an illness and/or injury; to determine a clinical diagnosis, treatment and appropriate discharge within their professional boundaries and in line with training, skills and competencies.

The post holder will be a source of clinical expertise to less experienced staff, enhancing the nurses skills and knowledge, and work in partnership within a multi-disciplinary team.

To contribute to meeting the identified Key Performance Indicators within the time frames identified.

To ensure a high quality patient centred service.

Job responsibilities and duties:

* To autonomously see and treat patients with minor illness and/or injuries.
* Complete annual reviews discussing treatment options and lifestyle modification for patients with chronic disease
* Take a comprehensive history from the presenting patient and/or accompanying carer/relative as appropriate
* Undertake a complete physical examination relevant to the presenting complaint
* Be able to formulate a differential diagnosis and order further appropriate investigations as required
* Be competent in interpreting results of investigations ordered e.g. blood tests
* Prescribe medication for patients as needed in line with the local prescribing guidelines
* Refer or arrange admission for patients on for further investigations/opinion as needed. Liaise with speciality teams as required.
* Liaise with other members of the multi-disciplinary team members as needed

Skills/Responsibilities:

Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council

Advanced Nurse Practitioner (RCN Accredited) Or Advanced Clinical Practitioner (RCEM Accredited) or equivalent qualification.

Experience in General Practice/ Walk-in Centre/ Urgent Care Centre/ Emergency Department/Out of Hours Service desirable

Evidence of teaching and mentoring junior staff

Evidence of undertaking clinical audits

Up to date knowledge of DOH and NICE guidelines

Knowledge of use of IT/library searches knowledge

Can discuss in depth current clinical and managerial issues regarding safeguarding adults/children and domestic abuse
