<div><p>We are seeking a skilled Registered Nurse to join our experienced Community Nursing Team. In this role, you will design and deliver personalised care plans to support people with complex health needs in their homes, helping them remain well, avoid unnecessary hospital admissions, and manage their health confidently.</p><h3>Main duties of the job</h3><p><b>What you’ll do:</b></p><ul><li>Complete holistic assessments and create tailored care plans that meet individual needs.</li><li>Act as a care coordinator, facilitating smooth transitions and effective communication across multidisciplinary teams.</li><li>Deliver clinical interventions, including wound care, IV therapies, and end‑of‑life support.</li><li>Monitor caseloads proactively and respond swiftly to changes in patients’ conditions.</li></ul><h3>What we’re looking for</h3><ul><li>A registered nurse with up‑to‑date clinical knowledge and a record of ongoing professional development.</li><li>Experience in community nursing or caring for people with long‑term conditions.</li><li>Strong communication skills and confidence working with patients, families and partners.</li><li>Ability to work independently, prioritise work and provide supervision to support colleagues.</li><li>Willingness to work towards non‑medical prescribing (V100/V150) if not already achieved.</li><li>Non‑medical prescribing V100/V150 or willingness to work towards it within a specified timeframe.</li><li>Specialist training or short courses relevant to the post, or equivalent experience.</li><li>Evidence of ongoing continuing professional development or a portfolio.</li><li>Understanding of current legislation, evidence‑based practice, caseload management, audit processes, KPIs, and restorative practice.</li><li>Experience of using IT in clinical practice and of planning and providing care for patients with long‑term conditions, wound management, and palliative care.</li><li>Proactive caseload management and experience acting as a care coordinator.</li></ul><p><b>We welcome applicants from all backgrounds and value people who bring fresh ideas, lived experience and a commitment to fair, respectful, and inclusive care.</b></p><p>CHCP CIC employees have access to an excellent range of benefits, including health and wellbeing programmes, career development opportunities and a supportive work environment.</p><p>You must have appropriate UK professional registration.</p><p>If you require any extra support, please let us know.
