Leicestershire Partnership NHS Trust is working together with Adult Social Care across Leicester, Leicestershire, and Rutland (LLR) to deliver our Home First service alongside core Community Therapy.

This is an exciting time to join the service, which brings together an integrated health and social care offer delivered by community-based nursing, therapy, and adult social care teams. You will use your therapy skills to:
1. Support people to remain in their homes during health or social care crises, reducing the need for hospital or care home admission.
2. Help patients get home from hospital quickly and provide rehabilitation and reablement to restore health, wellbeing, and independence.

You will deliver high-quality core therapy reablement, rehabilitation, and recovery services to people in their own homes.
Community therapy offers regular IST, development opportunities for clinical and leadership skills through training, working alongside dedicated Clinical Specialists and Operational leads, and course attendance. We also offer flexible and blended working arrangements.

Job Description Summary (Main Duties and Responsibilities)
We are seeking Band 6 Physiotherapists to provide an effective and efficient therapy service. This role requires a motivated, resilient, and proactive individual with personal responsibility, the ability to prioritize and manage their caseload, work autonomously, and as part of a team. Excellent communication and organizational skills are essential, along with the ability to adapt to a changing and pressurized environment. You will work with Occupational Therapists and Technical Instructors to carry out comprehensive functional assessments and interventions to promote patient independence.
The therapy teams operate in an integrated manner, encouraging synergistic working to achieve the best patient outcomes. You will be supported to develop your skills and challenge existing practices. We seek determined, self-motivated individuals who can make a positive impact on patient care and promote therapy services within the trust.
