Rehabilitation Assistant – Band 3
An opportunity has arisen for a Full‑time Rehabilitation Assistant to work within the ENable Community Neurological Rehabilitation and Falls Teams. These teams are part of the Maximising Independence service line within Ealing Community Partners (ECP).
Rehabilitation Assistants work as part of the multidisciplinary team, working closely with physiotherapists and occupational therapists. We provide rehabilitation in the patients’ home environment with a focus on promoting independence, meeting rehabilitation goals and reducing care needs in the community. Rehabilitation assistants are integral to running our patient groups in Southall and Acton.
This is an exciting opportunity for a health‑care support worker who already has some experience of working with patients in an acute or community healthcare setting to expand their skills and work within a friendly community team.
The post holder must be flexible to meet the needs of the Service and it is essential they have a current driving licence and access to a vehicle, as extensive travel across the Borough is required.

Responsibilities
* Work as a rehabilitation assistant across the ENable Team and the Falls Team, delivering care to people in their own homes, the local community and/or designated buildings such as care homes, Ealing day treatment centre, Clayponds Hospital, charity halls/community centres, with occasional support to other services in the maximising directorate.
* Assist therapists by carrying out delegated aspects of patient care such as practising exercises or reviewing equipment, with supervision as required, reporting back patient progress and informing qualified staff of any issues.
* Assist with equipment ordering and stock control for department supplies and ensure that these are received in full and good working order, following up on any outstanding items or actions as required.
* Undertake support work to aid the rehabilitation of patients in the community or the management of patients at home to prevent an unnecessary hospital admission.
* Assist with running various therapist‑led group exercise sessions such as upper‑limb exercise class, falls, balance & strength class and adapted tai chi exercise class.

Person Specification – Education
* Good level of general education (GCSE Maths and English).
* Evidence of continuous development.
Person Specification – Desirable Education
* NVQ Level 3 qualification in health and social care, or equivalent experience.
Skills and Abilities – Essential Criteria
* Able to communicate clearly and accurately, verbally and in writing, to a wide range of people including users, colleagues, carers, and managers.
* Basic IT skills – able to use a computer for emails, word and general internet use.
* Ability to work in a team, demonstrate flexibility and respond to changing demands and priorities.
* Able to drive and have use of a car for work or similar.
Experience – Essential Criteria
* Have knowledge and understanding of the role of Rehabilitation professionals in the community.
* Previous work experience, voluntary or paid, as a Rehabilitation assistant or similar Health or Care Assistant role in a social or healthcare environment.
* Experience of working with older people and people with long‑term conditions.
* Ability to and experience of working in a team.
Experience – Desirable Criteria
* Understanding the NHS and Social Care.
* Understanding the roles of physiotherapy and occupational therapy.
