Are you a clinical pharmacist looking for an exciting role with the hospital at home team supporting virtual ward, acute care at home, Community Treatment and Assessment Units and Urgent Treatment Centres? Are you looking for a role with variety focused on clinical patient care. Then this is the job for you!

This exciting role will provide clinical pharmacy services to Community Hospitals

You will be a core member of the Trusts urgent care team, providing professional pharmaceutical governance, medicines safety, medicines information, medicines finance, education and training and medicines optimisation support to the team and the services patients. You will build relationships and work with internal stakeholders and external partners to support the delivery of high-quality pharmaceutical care to all patients.

We welcome applicants from individuals who are hardworking and are motivated by wanting to optimise patient care and develop pharmacy services. Working closely with other healthcare professionals, including attending consultant ward rounds, working with specialist nurse teams and you will be part of a supportive and experienced pharmacy team.

You will be supported to develop your skill set with both the Nonmedical prescribing and the advanced practitioner pathway.

Main duties of the job

This role will include the development of a clinical ward service, with full support from experienced medicines management technicians. The service will involve; attendance at consultant ward rounds, encouraging uptake of the self-administration policy, education of members of the multi-disciplinary team and counselling patients and carers on medication.

In addition, you will be responsible for development and review of suitable polices and guidelines to support the expanding role of the Urgent Treatment Unit. This is an excellent opportunity for anyone with a passion for service development whilst maintaining and expanding their clinical skill set. You will line manage the pharmacy staff working in your area and you will have the opportunity to develop managerial skills.

Person specification Qualification Essential criteria

* Professional Qualification MSc in pharmacy or equivalent (BSc, BPharm)
* Current professional registration i.e. GPhC
* Evidence of Continuous Professional Development
* Independent prescriber* *if not completed or currently undertaking must be willing to complete at earliest convenience
* Post-graduate Diploma in Clinical Pharmacy or equivalent

Skills and Aptitude Essential criteria

* Excellent written and oral skills and able to communicate complex information effectively at all levels. Have the appropriate IT skills to utilise clinical information systems, databases, and other software to improve patient care
* Able to critically review, analyse and interpret complex clinical literature and medicines legislation
