As a Mental Health Practitioner you will be providing specialist mental healthcare to offenders and working as part of the Mental Health In-Reach Team and the wider mental health services. You will work in a psychologically minded way with offenders to achieve their agreed goals and quality health outcomes.
Responsibilities
* Provide specialist mental healthcare to service users and work as part of the Mental Health In-Reach Team and wider MDT.
* Deliver a range of specialist interventions at primary and secondary care level, including referral management, screening, assessment, triage, and evidence-based interventions.
* Care planning and risk assessment; one-to-one and group-work facilitation.
* Manage a mixed and challenging caseload, including offenders with learning disabilities and mental health conditions; contribute to alternatives to inpatient admission and support early discharge through high-intensity interventions and complex case management.
* Work closely with community mental health teams to ensure information sharing, continuity of care and the Care Programme Approach (CPA) where necessary.
* Provide comprehensive and timely reports and mental health expertise to multi-agency forums (e.g., MAPPA), police, probation services, solicitors and court liaison teams as directed.
* Deliver clinical leadership and supervision within the team; participate in supervision as directed.
* Participate in resource centre services; deliver psychological therapy and specialist activities under the direction of the clinical lead.
* Develop a specialist lead role as appropriate and provide peer support and training in this area; contribute toward support, advice, consultation, and training for prison staff.
* Contribute to sentence planning, resettlement, safer custody and ACCT processes through close collaboration with prison staff.
* Engage in training to maximise uptake of screening, health promotion and prevention activities, including vaccinations.
* Coordinate single, integrated care plans with service users and other agencies; ensure comprehensive risk assessment, crisis plans and rapid access plans are in place and regularly reviewed.
* Attend and contribute to weekly MDT meetings for service users on secondary caseload and complex care needs.
* Support patients to manage all areas of their health, from general health promotion to long-term condition management; embed health promotion in every aspect of the service.

All applicants must be willing to undertake National Security Vetting to work in a prison setting.
