North East | £30 per hour | Hybrid/Field-Based Role

About the Role
As part of the Adult Social Work Team, you’ll handle complex cases, complete Care Act Assessments, and contribute to safeguarding processes. You'll also collaborate with the Integrated Hospital Discharge Team at Scunthorpe General Hospital.
You'll apply legislation such as the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act to deliver safe, person-centred support that promotes independence and wellbeing.
Key Responsibilities
1. Conduct Care Act Assessments in community and hospital settings
• Lead safeguarding inquiries and manage complex caseloads
• Complete Mental Capacity Assessments and Best Interest decisions
• Develop personalised, empowering care plans
• Collaborate with clients, carers, and partner agencies
• Support unqualified staff and mentor social work students
About You
We’re looking for someone who is:
• A Qualified Social Worker with Social Work England registration
• Experienced (3+ years) in Adult Social Care
• Knowledgeable in adult legislation and safeguarding practices
• Skilled in communication, mediation, and complex decision-making
• Flexible, empathetic, and focused on outcomes
• Confident working both independently and within a multi-disciplinary team
