Job Title: Adult Social Worker – Hospital Discharge Team Location: Hackney, London Contract: Full-Time – 36 hours per week - Initiall 3 month contract - potential to be extended Rate: £32 per hour (Umbrella) £28 PAYE About the Role We are currently recruiting for an experienced Adult Social Worker to join the Hospital Discharge Team on behalf of Hackney Council. This role plays a key part in supporting safe and timely hospital discharges, ensuring individuals receive the appropriate care and support when returning to the community. You will work in a fast-paced, multidisciplinary hospital environment, collaborating with healthcare professionals, families, and community services to assess needs and coordinate effective discharge planning. Key Responsibilities Undertake strengths-based assessments to support safe hospital discharge Develop and implement person-centred care and support plans Work closely with NHS colleagues, discharge coordinators, and community services Facilitate timely discharges while ensuring appropriate care packages are in place Undertake safeguarding enquiries where required Apply legislation including the Care Act 2014 and Mental Capacity Act Maintain accurate case recordings and documentation in line with statutory requirementsRequirements Qualified Social Worker with Social Work England registration Experience working within adult social care, ideally within a hospital discharg...
