We are seeking an experienced Clinical Pharmacist who is an Independent Prescriber to support a service in Preston. This position requires an individual who is confident working autonomously and has current clearances related to drug misuse services and secure environments. The successful candidate will play a key role in delivering high quality medicines management, prescribing support and patient-centred care within a busy clinical setting.
Key Responsibilities:
Provide clinical pharmacist support including medication reviews and medicines optimisation
Prescribe safely and appropriately as an Independent Prescriber
Manage patients with long term conditions and complex medication regimes
Work closely with multidisciplinary teams to ensure effective clinical pathways
Maintain accurate prescribing and clinical documentation
Essential Requirements:
Fully qualified Clinical Pharmacist
Independent Prescriber (IP) qualification
Drug misuse clearance
Prison clearance or eligibility to obtain it
Strong clinical decision making and communication skills
