Discharge Liaison Officer
The primary focus of the Discharge Liaison Officer role is to enable and support the safe and timely discharge of patients. The post holder will need to consider the options around a patient’s discharge and take appropriate action to resolve delays. This ensures capacity is created in the system, so wards always have the beds they need.
Key to the role is communication with Bed Managers and the nurse in charge, ensuring everyone is working to the same plan and that no patients experience delays.
The post is part of Bronze Command (BC) and works with staff at Scarborough Hospital, Vale of York CCG, Scarborough and Ryedale CCG, East Riding CCG, City of York County Council, North Yorkshire County Council and East Riding County Council, as well as patients and their carers. The role is on a rotational basis, with the ward changing as determined by the Patient Flow Manager (PFM).

Responsibilities
* Identify and address discharge delays, working collaboratively with ward staff, social care, continuing health care and the voluntary sector.
* Coordinate with multidisciplinary teams to optimise patient discharges from acute medical, surgical or elderly wards.
* Communicate discharge plans to Bed Managers, nurses and patients, ensuring clear, timely action.
* Act proactively, remaining flexible and maintaining a "can do" attitude.
* Support the Patient Flow team in maintaining system capacity and meeting demand.

Relocation Package
The Trust offers a generous relocation package of up to 30% of salary (capped at £18,000) to support relocation costs for positions where relocation would be an expectation to undertake the role fully.
