Job Summary
This position provides person-centered case management services to individuals returning to the community from incarceration, as part of Delphi Rise’s Monroe County Reentry Program. Our approach is grounded in dignity, respect, and belief in each person’s potential. We walk alongside clients as they rebuild their lives, offering support with housing, employment, health care, and healing.

Essential Duties and Responsibilities
Additional duties may be assigned.
* Manage a caseload of 40 to 50 clients, supporting access to benefits, employment, treatment, housing, childcare, healthcare, identification documents, and other critical services.
* Coordinate services in collaboration with the New York State Department of Corrections and Community Supervision (Parole), Monroe County Probation, local courts, and community partners.
* Maintain accurate and timely case notes and client records.
* Develop individualized service plans that align with each client’s goals and needs.
* Facilitate or arrange support groups such as Cognitive Behavioral Therapy, parenting education, domestic violence support, and anger management.
* Conduct biweekly follow-up contacts with all assigned clients.
* Actively participate in Monroe County Reentry Task Force meetings.
* Provide services on-site at the Parole Office one to two days per week.
* Perform other duties as assigned by management.

This role does not include supervisory responsibilities.

* Familiarity with community resources that meet diverse client needs.
* Ability to work independently while contributing positively to a team environment.
* Cultural humility and comfort working in diverse environments.
* Adaptability and a good sense of humor.
* Proficiency with computers, including web-based data systems.
* Valid driver’s license with a clean driving record. Occasional travel required for meetings, appointments, and community events.

Moderate attention required for administrative tasks, data entry, and client documentation. Some analysis and decision-making needed within established procedures.
Physical Requirements
The role involves frequent sitting, standing, walking, talking, hearing, and repetitive hand motions. Occasional pushing or pulling. No heavy lifting (under 5 pounds).
Environmental Conditions
Some off-site work may be required to meet with clients or attend community events. This may include exposure to inclement weather or travel to partner agency locations.
