Main duties of the job

The Care Coordinator will work with the PCN team and member practices to coordinate and perform key activities to manage patient caseloads, provide access and navigation of care and support across health and care services, and improve outcomes for patients.

The post-holder will perform patient call/recall, appointment bookings and proactive reminder calls, construct and run SystmOne searches to identify patients for recall, ensuring invites are made in a timely and appropriate manner via text, telephone or letter and accurately coded within patient medical records. Other duties will include but are not limited to audits, data entry and reconciliation, data reporting and data cleansing.

Previous experience working within the NHS is desirable for this role. Opportunities for professional development are included within this role.

Qualifications

* Demonstrable commitment to professional and personal development is enrolled in, undertaking or qualified from appropriate training as set out in the core curriculum by the Personalised Care Institute, or prepared to undertake training
* Good Standard Education


Experience

* Experience working in Primary Care Network or NHS practices System One experience
* Demonstrable experience of effectively engaging with key stakeholders, such as clinical staff
* Experience of working within multi-professional team environments
* Experience of working under pressure to tight deadlines
* Experience of operating in a politically sensitive environment


Skills and Abilities

* Excellent communication skills (written and oral)
* Good knowledge of Microsoft office products
* Self-awareness and good people skills
* Excellent organisational and time management skills
* Excellent organisation and time management skills with an ability to prioritise and meet deadlines
* Ability to adopt a proactive approach to the role
* An understanding of the bodies in the wider Health system such as STPs, ICSs, NHSE/I, CQC, etc
