Job Overview
An exciting opportunity to join our organisation as a Deputy Community Nurse Team Lead for the Watergate Community Nursing Team. As an autonomous practitioner you will be responsible for the assessment and treatment of patients with complex health and social care needs. You will manage a clinical caseload within a community nursing team with guidance from the Community Matron, Community Nurse Team Leads and Integrated Care Team Leads. You will ensure provision of a holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes. This includes physical examination, detailed history taking, diagnosis of deterioration in pre-existing conditions and treatment planning.
Main duties of the job
* This is a community-based role that requires frequent travel to various locations throughout the county. Successful candidates will need to demonstrate how they will manage these travel requirements.
* Work in an adaptable and evolving role, focusing on safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings.
* Implement personalised support plans and collaborate with the whole community resource to facilitate ongoing care management.
* Work in partnership with patients, carers and the multi-disciplinary team to offer support and advice, promoting informed choice; maximise independence and quality of life.
* Provide clinical management and leadership to all staff within the team.

Person Specification
Education / Qualifications (Essential)
* Current professional registration RN (Adult)
* Evidence of recent relevant professional development
* A degree in nursing
Skills and Aptitude (Essential)
* Leadership skills to motivate a range of staff, organise self and others, delegating and prioritising workload
* Conflict resolution skills
* IT skills including Word, Excel, spreadsheets, email and internet access
Desirable
* Advanced computer skills
