Overview
This exciting post will be working in the Memory Assessment Service (MAS) providing assessment, potential diagnosis and signposting to other services and support networks for individuals aged 65 and over referred to the service with cognitive impairment. We are looking to recruit a Band 6 Memory Assessment Nurse. The posts will work in the areas with the highest waiting times and will work with colleagues to reduce the waiting times in that area. There will be a requirement to travel between sites throughout Nottinghamshire.
Responsibilities
* To work as part of the multidisciplinary team to screen for appropriate referrals
* To complete an assessment of clinical presentation and symptoms
* To share information amongst the multidisciplinary team and create appropriate and person centered care plans
* To refer for appropriate diagnostic procedures e.g. scans, neuropsychology, occupational therapy
* To make independent diagnostic decisions based on consultation, history, presentation, pathology and/or imaging results and to seek supervision from service medical support in complex diagnostic situations
* To complete accurate records
* To screen referrals for appropriateness
* To undertake initial assessments and create a treatment plan
* To liaise with the MDT to explore symptoms and causes
* To refer for additional assessments

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. A full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
