Job description
The TRI team offers a short-term, therapy-led, TEC-first reablement service for individuals within hospital discharge pathways and the community. This role involves managing a varied caseload, providing case management and specialist physiotherapy input in collaboration with domiciliary care providers and care home staff (residential reablement).
Responsibilities include, but are not limited to:
* Providing comprehensive case management and specialist physiotherapy services.
* Working alongside domiciliary care providers and care home staff to deliver residential reablement.
* Conducting specialist assessments, including falls risk assessments, and developing goal-oriented discharge plans.
* Supplying necessary equipment to support reablement and independence.
* Implementing specialist interventions to enhance patient outcomes.
Requirements:
* Degree in Physiotherapy.
* Registered with the Health and Care Professions Council (HCPC).
* Demonstrated experience in adult physiotherapy, preferably within reablement or community settings.
* Awareness of Technology Enabled Care (TEC) devices or a willingness to learn and incorporate these into practice.
* Strong assessment and clinical reasoning skills.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a multidisciplinary team.
* Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
