An interesting and exciting opportunity has arisen for a highly motivated and enthusiastic individual to join the Urology department at Nottingham University hospitals as a Cancer Care Co-ordinator & Pathway Navigator .As the Cancer Care Co-ordinator & Pathway Navigator, the successful candidate will play a key role in helping with the delivery of cancer targets for the trust by ensuring that the journey of each cancer patient runs smoothly and is progressed in a seamless and timely manner.
Main duties of the job
To support the Urology team in delivering a high standard of support to patients with cancer. This may involve contact with patients. The post-holder will be expected to support and signpost patients over the telephone and progress their care pathway in liaison with other colleagues and departments in the hospital. The post-holder will also be expected to be involved with Audit and Peer Review processes.
The post holder will need to be flexible and self-motivated with a good attention to detail and the initiative to work independently as well as a team. This post is key in providing high quality information and ensuring patient's progress effectively on the cancer post holder will work closely with staff across the wider trust including clinicians, general managers, deputy managers, nurses, secretaries and other administrative staff.

1. To work as a member of the Urology team and providing patient coordination and care. The post holder will support ensuring a smooth and efficient service for patients by navigating and signposting them throughout their complex timed cancer pathway. Navigation will commence from the point of referral for suspicion of cancer and will support the cancer service in implementing and achieving the timed cancer optimal pathway, to achieve definitive diagnosis by day 28.
2. Document and monitor all aspects of patient care coordination and service delivery, supporting data collection for audit. The post-holder will work closely with Cancer Multidisciplinary Services to support and coordinate the care of cancer patients through their complex care pathways, ensuring all patients move through the pathway without unnecessary delays leading to quicker diagnosis and treatment, reducing patient cancellations and DNAs (Did Not Attends), therefore leading to a quicker diagnosis and treatment pathway.
3. Collaborate and communicate with the CNS and wider Multi-disciplinary Team to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of health care delivery.
