An exciting opportunity has arisen to join our Children in Care (CiC) team based at Sandwell Health Campus within the Safeguarding Children’s Team and co-located Sandwell Children's Trust, Wellman Building, an innovative development which has proven positive for partnership working and information sharing with CiC social workers.
This is a diverse role which entails working closely with children, young people, their carers and multiagency partners offering clinical advice, delivering health education and completing statutory Review Health Assessments (RHA). The role incorporates undertaking RHA’s for Sandwell’s CiC population who live within a 50 mile radius of Sandwell and are completed within the home, school or clinic setting.
You will also be expected to work alongside other health professionals, and partner agencies to promote and drive forward the national and local agenda for the health of looked after children. The post holder will work jointly with Named and Designated professionals to maintain a co-ordinated approach to the improvement of outcomes for Sandwell’s children in care and young people. Innovative working and gaining the views of children and young people on the services they are offered are essential to the role.
If you are a registered nurse with SCPHN/community paediatric experience we would like to hear from you. Experience of working with children and young people with complex health and social needs, including emotional health difficulties and learning difficulties, would be advantageous. However, a passion for working with, and improving the services for, children in care and young people is a necessity for the successful candidate.
Key Duties
* To communicate effectively with children, young people, team members and staff from other agencies (for example teenage pregnancy, youth offending team and substance misuse services).
* To communicate and negotiate with other health care professionals to undertake Looked After Health Assessments for young people placed out of borough (up to 50 mile radius).
* To communicate effectively with Health Visitors and School Health Nurses to ensure the particular health needs of vulnerable young people are met.
* To support foster carers to promote the benefits of a healthy lifestyle and to encourage them to be active partners in supporting children and young people.
