We are looking to recruit a Health Care Support Worker to join our North Connect PCN based in Northwood on a 6 month fixed term contract.
The role will be based at Devonshire Lodge Practice in Pinner.

Job Description
Responsibilities
* Supports health promotion and prevention activities across the PCN, including smoking cessation, weight management, and lifestyle interventions
* Plays a key role in vaccination and immunisation programmes within the PCN
* Works within a multidisciplinary team to support patients with long‑term conditions and health inequalities
* Provides clinical support to registered nurses and other healthcare professionals within the PCN
* Delivers basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives
* Takes and records blood pressure, height & weight
* Performs ECGs and phlebotomy
* Assists with chronic disease monitoring
* Conducts health checks
* Assists GPs and nurses with minor procedures
* Updates patient records and ensures accurate documentation
Personal/Professional Development
* Post holder must hold a care certificate or work towards one
* Participates in an annual individual performance review, taking responsibility for maintaining a record of personal and/or professional development
* Takes responsibility for own development, learning and performance and demonstrates skills and activities to others who are undertaking similar work
* Informs the lead GP of any concerns regarding the role and any professional development needed
* Is aware of own professional boundaries and what to do when they are reached

Person Specification
Qualifications – Essential
* GCSE Grade A‑C in English and Maths
* Has or is willing to work towards a care certificate or equivalent qualification

Qualifications – Desirable
* Full driving licence
Experience – Essential
* Experience of coordinating tasks and liaising with multiple healthcare professionals both internal and external to the organisation

* Primary care experience as a healthcare assistant, practice administrator, or other
* Experience of working with healthcare professionals and/or previous experience in the NHS, social care or a relevant field
* Experience of using clinical systems such as EMIS
