This full-time role is based in beautiful Richmond and involves care coordination, rota planning, mentoring, and daily communication with clients and carers. About The Role
As a Care Coordinator, you will:
* Plan and manage staff rotas using advanced scheduling systems to ensure seamless care delivery
* Build strong, trusted relationships with clients, carers, and colleagues
* Support and mentor carers, helping them deliver high-quality, person-centred care
* Work closely with the care manager to ensure care is aligned with each client's clinical and personal needs
* Collaborate with recruitment and HR to onboard staff and train them in our systems
* Join our shared out-of-hours rota, supporting the team when unexpected issues arise
* Be a key part of a passionate and responsive team, solving challenges together
