Overview

North HampshireUrgent Care are introducing a new primary care service within the NorthHampshire area. This service will provide a day time Paramedic and ACP-led homevisiting service for patients within four Primary Care Networks (PCNs)presenting with unplanned acute and subacute primary and urgent care needs. Weare looking for an experienced ACP with experience of working within primarycare to work as part of a team with non-prescribing home visiting Paramedics,providing support and guidance as required.

The service willbe operating 10:00-18:00 Monday to Fridays (excluding bank holidays) and wewould be interested from hearing from those who are looking for either fulltime or part time roles.

The successfulapplicant will be required to demonstrate the NHUC Values, have goodorganisational skills, and be competent in using IT tools and systems. Theymust be able to manage changing demands and priorities and work effectively asboth part of a team and independently. Experience of working in a similarenvironment would be beneficial but not essential.

Please note, this is a fixed term position which is subject to final contract confirmation.

Main duties of the job

The successful candidate will be autonomously assessing, diagnosing and discharging or referring patients within their scope of practice with the support of GP practice colleagues and remote support provided by NHUC clinicians as required. The successful candidate will also be streaming inbound referrals and allocating these between the home visiting team operating on the day, or contacting the patient remotely and escalating to alternative services if clinically appropriate. We are looking for highly motivated and enthusiastic individuals who will help support the new service and build relationships with the practices and PCNs that the service is operating within as part of the role.

About us

North Hampshire Urgent Care Ltd (NHUC) is a Community Benefit Society, working in partnership with the NHS. NHUC provides services through two divisions, Same Day Care (SDC) and TalkPlus.

SDC provides services through the Integrated Urgent Care (IUC) contract which covers the in Hours Clinical Assessment Service (CAS) and Out of Hours (OOH) provision for North Hampshire, North East Hampshire and Farnham (NEHF) and Surrey Heath places which are part of the Hampshire and the Isle of Wight ICB and Frimley ICB respectively. NHUC also provides Urgent Treatment Centres at the front doors of both Basingstoke and Frimley Park Hospitals whilst also holding some smaller contacts supporting Community Hospitals OOH and Flu response. Finally, SDC has a virtual care arm supporting patients at home.

TalkPlus provide NHS Talking Therapies, supporting people with common mental health problems in North East Hampshire and Farnham place. TalkPlus offers a range of treatments to people who are experiencing symptoms of stress, anxiety, low mood, depression, panic, phobias or OCD. TalkPlus has care pathways to provide timely treatment for perinatal and military veteran referrals.

Job responsibilities

Role Summary

To work as part of the primary care home visiting team, providing acute assessment, investigations and diagnostics of unplanned acute and subacute urgent and primary care conditions to housebound patients.

The post holder will be responsible for providing appropriate urgent primary care level treatment and either discharging or onward referring patients as appropriate within their scope of practice with the support of remote clinical supervision. The post holder will also provide triage and remote supervision support for other members of the primary care home visiting team.

Main Duties and Responsibilities

Clinical Care

Act as an expert practitioner, autonomously undertaking visits to patients home addresses, providing assessment and diagnostics as required of acute and subacute primary and urgent care referrals.

Demonstrating clinical competence and a knowledge base supplementing those associated with conventional undergraduate roles.

Undertake an element of triage, streaming patients to the most appropriate home visiting practitioner, or escalating or redirecting patients to alternative services when clinically appropriate.

Undertake face-to-face consultations with patients as an autonomous practitioner and using own clinical judgement, diagnose, treat, refer and or discharge patients within competence.

Undertake holistic assessment of wider care needs as appropriate.

Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.

Ensure that patients receive high quality clinical care, delivered in a timely manner.

Be an independent prescriber and safely supply and administer medicines.

Provide clinical advice to Administration staff where required.

Ensure complete and accurate documentation of each and every patient contact.

Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met.

Ensure that professional practice adheres to organisational policies, procedures and guidelines.

Develop a professional partnership relationship with the GP surgeries utilising the home visiting service.

Communicate effectively with patients and families regarding complex information about their care

Provide advice and information to patients, carers and their families where appropriate

Communicate effectively and work collaboratively with medical, nursing and allied health professionals to ensure delivery of a co-ordinated service.

Ensure accurate documentation record keeping in line with local policies and HCPC/NMC guidelines.

Promote and demonstrate effective communication networks within the organisation, with partner organisations and other service providers

Clinical Governance

Cooperate with all investigations and audits as required.

Adhere to North Hampshire Urgent Cares policies, procedures and guidelines.

Service Improvement

Contribute to quality improvement within the service through the identification and initiation of changes, which lead to better evidence based practice and improved standards of care

Participate in clinical audit identifying areas of best practice and areas for improvement.

Contribute to the setting and monitoring of standards of care.

Help develop the services available to the urgent care population to reflect identified needs

Educational/Personal Development

Contribute to the development and maintenance of an excellent learning environment.

Support clinical practice and development of knowledge/skills.

Adhere to professional registration requirements to maintain professional development, participate in supervision and revalidation in line with HCPC/NMC guidance.

Undertake and complete all statutory and mandatory training, in line with NHUC requirements in particular for Basic Life Support/Intermediate Life Support, Information Governance, Infection Prevention Control, Safeguarding and conflict resolution.

Person Specification


Skills

* Demonstrate excellence in the ability to assess and manage patient risk effectively and safely

Knowledge

Qualifications

* HCPC Registered Paramedic or NMC Registered Nurse
* Full MSc qualification in a related field OR recognition of Advanced Practice from the Centre of Advancing Practice

Experience

* At least five years clinical experience working operationally in either a primary, urgent or emergency care setting
* Experience of providing whole episode of care to patients including independent prescribing, across all age groups including children and elderly
* Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge
* Experience of autonomously assessing, diagnosing and managing a wide range of acute and subacute presentations, together with identifying holistic patient needs in a short timescale
* Evidence of working autonomously and as part of a team
* Proven ability to evaluate the safety and effectiveness of their own clinical practice
* Evidence of knowledge of Paramedic undergraduate training to enable the support of B7 Paramedics operating in the service.
* Experience of working in a Primary Care, Community, Triage and/or A&E setting
* Evidence of leadership and mentoring skills
* Experience of engaging external stakeholders
