Although the post holder will be an integral part of the Social prescribing team, they will report to the Community Matron via the Social prescribing link workers and community Midwives. They will assist in providing an efficient, effective, and comprehensive reception, administrative service for the multidisciplinary team of healthcare professionals.
Using appropriate technologies and equipment, and taking guidance from the Admin Team Leader, the post holder will produce work outputs to a high standard that meets the organisation's requirements for patient care, safety, Clinical Governance, Information Governance, RMS, and CQC, in line with all policies, procedures, and guidelines.
The post holder will assist in delivering a professional, high-quality, effective, and efficient administrative and clerical support service to a multidisciplinary team, meeting the needs of the service and complying with all relevant policies and procedures. They will work under tight deadlines while remaining calm, flexible, and organised.
Main duties of the job
1. Undertake daily administrative and clerical duties specific to the allocated area to support the care of women with complex social needs, following guidance from the Matron and Midwifery and Social prescribing team to ensure patient safety and confidentiality.
2. Handle queries from service users, including key stakeholders, charitable organisations, GPs, patients, and their representatives, professionally and courteously, in accordance with legislation, policies, and procedures. Source relevant information, take accurate messages, and escalate complex queries as needed.
3. Ensure all work is completed to a high standard, meeting requirements for patient safety, information governance, data protection, and confidentiality.
4. Liaise with the multidisciplinary team and external stakeholders, maintaining professionalism and confidentiality at all times.
5. Assist in locating, collecting, and returning healthcare records using appropriate technology, following tracking procedures and guidelines.
6. Coordinate appointments and correspondence efficiently.
7. Securely file all patient-related information, ensuring availability at the right time and place.
8. Input and verify data into trust IT systems, following policies and guidelines.
9. Organise patient referrals to other departments or hospitals for investigations or treatment.
10. Maintain an up-to-date filing system for work documentation and establish an effective follow-up system for responses and documents.
