About the role
As Team Leader at Greensand House Care Home in Surrey, you will support the home management team in delivering high‑quality care by creating a safe, caring, innovative, and inclusive environment for residents, visitors, and staff.
Responsibilities
* Lead the person‑centred care planning process, including implementation, monitoring, monthly evaluations, care‑plan audits, and action‑plan development and completion.
* Assist in the smooth transition of any discharges, handling documentation, medication, and communication with next of kin.
* Monitor residents’ well‑being, updating care plans to reflect changing needs and ensuring written records are amended accordingly.
* Practice safe systems of work across all tasks, assess risks, and maintain the safety of residents, visitors, and staff during moving and handling.
Qualifications
* NVQ 3 or equivalent in care for care homes.
* Minimum two years experience in a senior position within a care home.
* Strong verbal and written communication skills.
* Demonstrated ability to lead staff and act responsibly in the manager’s absence.

* ADBS disclosure check (cost covered by Avery).
* Proof of eligibility to work in the UK.
