This is a new post joining a team of 7 Consultants responsible for the provision of Acute Frailty and Eldery Care services to the Health Board.
We are looking to appoint three dynamic consultants committed to improving the care of older people in Swansea and Neath Port Talbot.
Swansea Bay University Health Board is transforming its acute medical services. This post will help deliver a new model of integrated care for older people within Swansea Bay University Health Board.
The post holders will be joining a team of Consultants in Swansea responsible for the provision of geriatric medical services to the Health Board.
The posts will have clinical responsibilities, supported by resident doctors, for the care of acutely unwell older people living with frailty in a variety of settings depending upon the individual posts and the interests of the post-holders.
The post holder will:
1. Participate in delivering our acute frailty services. This will include running the frailty single point of access integrated hub, overseeing and directing community services to avoid medical admission where possible. The consultant will be supported by a team of Advanced Clinical Practitioners (ACPs) and will direct patients to the appropriate community service (currently virtual wards and acute clinical teams). The Health Board is working towards greater integration of community services with a Hospital at Home (H@H) model supporting the acute frailty intake.
2. The job plan will include community sessions.
3. Consultants will rotationally look after patients on the Acute Frailty short stay unit and inpatient Geriatrics wards.
4. There will be the opportunity with agreement for one session to be used to pursue a relevant sub-speciality.
5. Be expected to implement and adhere to the SAFER bundle of care
6. The post holder will contribute to the general medical intake (and same day emergency care if desired).
7. Contribute to the department's strong record in undergraduate and post graduate training.
8. Contribute and participate in the department's strong record in quality improvement work and research.
The person appointed will have broad training in Geriatric Medicine and General (Internal) Medicine and have experience of multidisciplinary working in acute and rehabilitation settings. The appointee will be capable of managing the range of medical problems that occur within the setting of a general hospital and an understanding of older people living with frailty in the community. The appointee must be an effective team player and will be flexible in approaching new ways of working.
Applicants must have MRCP (UK) (or equivalent specialist qualification) and hold a Certificate of Completion of Training (CCT) in Geriatric Medicine and General Internal Medicine, or be within six months of award of CCT, or equivalent by date of interview.
Applicants must also be on the Specialist Register of the General Medical Council (UK) with a licence to practice (or be eligible for full and specialist registration within six months of interview).
