Chapel Street Surgery in Newhaven is a medium sized (5,000 patient) practice in a deprived area that is being regenerated. The practice has been through some difficult times, but we are now on the way up. We have improved from a significant turnover of staff, CQC rating of Requires Improvement and poor premises to: A fully staffed nurse/nurse associate/HCA team A newly-appointed practice manager A fresh pharmacy/technician team An expert Advanced Practitioner team (one paramedic and two nurses) A fully staffed admin/reception team New premises – we’ll move in the summer of next year CQC Rating of Good. The practice is part of the Sussex Primary Care group, so we have access to a wide range of peer support and expertise and the opportunity to work in SCFT services as part of a portfolio career. Our vision is to provide excellent, innovative primary care to give our patients the best possible health and social functioning. We’ll do this by combining the best of the traditional Family Doctor continuity of care with modern approaches to multi-disciplinary working, use of digital technology and community integration; we want our patients to be partners with us to take control of their own health. We aim to be a teaching and training practice within 3 years. We need two or three GPs, working 12 sessions between them, to complete the team and bring the energy and drive to make this vision a reality. We can be flexible with this – especially when it comes to portfolio working. Here are some changes we have recently brought about: Total Clinical Triage, whereby all incoming requests from patients are scrutinised by a senior clinical decision-maker and allocated to the appropriate intervention. Overhaul of admin and prescribing systems to improve safety and reduce the burden on our doctors. The purpose-built new surgery premises we are moving into will provide space to co-locate other community services alongside us (in line with the national Ten Year Plan) and realise the ambition of optimising the local community’s mental and physical health.
