Urgent Treatment Centre Advanced Clinical Practitioner
We are looking to appoint a dynamic and forward‑thinking Qualified Advanced Clinical Practitioner (ACP) to work within our Urgent Treatment Centres based at Penrith Hospital with cross‑site working at Keswick Urgent Treatment Centre, in varying shifts between the opening hours of 08:00 and 22:00.
The post holder will be an experienced health care practitioner with leadership skills who will work autonomously in caring for patients with a range of urgent health problems whilst maintaining and developing their own advanced clinical assessment skills, diagnostic skills and understanding of investigations. They will manage full episodes of care including assessment of a range of systems, using clinical knowledge to formulate diagnosis, apply critical decision making in planning care and treatment plans and, where appropriate, work collaboratively with medical and wider teams outside the UTC to meet the needs of patients, supporting the delivery of policy and procedure and using national and local guidelines to ensure care is based on sound evidence. Provide an environment supportive to learners of all levels.
Main duties of the job
- Independently assess, differentially diagnose, plan, implement and evaluate individualised treatment plans for patients, resulting in safe appropriate management, referral or discharge without direct medical support on site.
- Prescribe independently (where legally permitted) within scope of competence and within the Royal Pharmaceutical Society Competency Framework, national and locally adapted guidelines and best available evidence for a range of urgent/acute presentations appropriate to patients need and desired therapeutic outcomes.
- Provide clinical care on priority of patient need and within the time constraints of the department opening hours including ensuring waiting targets are met. This entails managing patients who self‑refer to the service and also through referral pathways from other teams such as NHS 111, GP, community pharmacy, community staff, other hospitals, CHOC etc.
Person Specification
Qualifications
- Current registration with a professional body e.g. NMC, HCPC or GPhC
- Completed Master’s degree qualification (or equivalent prior to MSc being available e.g. Nurse Practitioner Degree, OSCEs and non‑medical prescribing)
- Independent prescriber
- Advanced clinical practice skills and clinical treatment skills
- Evidence of substantial clinical practice at advanced level in portfolio
- Evidence of competence within all core ACP competencies (as per DHFT ACP framework)
Experience
- Experience in managing acute problems
- Clinical diagnostic skills including the ability to interpret radiology and laboratory investigations
- Evidence of a teaching/coaching/ mentoring course
- Substantial broad experience relevant to clinical area
- Experience in leadership and/or management
- Evidence of competence within all core ACP competencies
- Has the ability to use basic IT systems including Word, Excel and PowerPoint, as well as ability to learn Trust specific IT systems
- Broad and advanced knowledge of clinical pathophysiology
- Has a sound level of knowledge in relation to invasive and non‑invasive diagnostic and therapeutic procedures
- Ability to take a full systematic focused history
- Ability to make a diagnosis based on interpretation of imaging and pathology results
- Has the knowledge to support safe clinical decision making in relation to discharge
- Willingness to perform and or learn advanced clinical skills appropriate to area of practice
- Ability to manage complete episodes of care and prioritise workload
- Experience within acute care
- Experience in managing chronic health issues
- Experience of minor injury assessment/ management, to include X‑ray interpretation
Knowledge
- Evidence of continual CPD aligned to a professional body, e.g. Royal College, or academic institution
- Advanced clinical assessment and diagnostic skills
- BSc/MSc level therapeutics or pharmacy related knowledge and non‑medical prescribing
- Highly motivated and experienced practitioner
- Physically able to perform all requirements of the ACP role where highly developed skills and accuracy are required which may include: suturing, IVI drug administration, central line care, applying plaster casts, inserting catheters and other specialist procedures relevant to area of practice
- Willingness to perform and or learn advanced clinical skills appropriate to area of practice
- Ability to manage a team of patients and prioritise workload
- Negotiating skills
