Hywel Dda Medicines Optimisation Team is seeking an enthusiastic and forward-thinking Advanced Pharmacist, to help drive innovation and excellence in patient-centred care across primary care settings. As an Advanced Pharmacist, you will support the coordination and delivery of a patient-focused clinical pharmacy service, working within the primary care team to further develop the generalist pharmacist role. This includes pharmacist-led clinics as an independent prescriber (IP), close collaboration with multidisciplinary teams, monitoring prescribing practice and financial expenditure, providing clinical mentorship and leading on service evaluation and improvement initiatives.
You will contribute to the implementation of medicines optimisation and provide a specialist, professional service that upholds high standards of patient care and safety. Main duties of the job
You will:
* Provide expert clinical and professional pharmacy services to patients in primary care, demonstrating high levels of expertise and skills across the four pillars of advanced practice
Apply in-depth knowledge and skills, whilst providing clinical services to individual patients or patient groupsrequiring complex care.
Provide highly specialist advice and guidance to members of the multi-disciplinary team (MDT) within primary care and across the interface with secondary care
Work collaboratively with others using negotiating skills to progress changes in prescribing patterns, which may be contentious and to overcome resistance to change in delivering cost effective and value-based solutions
Monitor prescribing activity and expenditure within area of responsibility, contributing to budget management and financial stewardship.
Support improvements in cost-effective and prudent use of medicines, utilising and analysing data, ensuring patient centred holistic care and management of clinical condition.
Support the development of education and training provision, to support the growth of others and advance practice.
Evaluates and identifies personal learning needs, creating a tailored development plan to enhance clinical skills and knowledge to expand scope of practice, while also fostering leadership abilities and the capacity to mentor others.

Person Specification
Qualifications and Knowledge
* Master's degree in Pharmacy
* Registered Pharmacist with GPhC.
* Post-graduate diploma/MSc in clinical pharmacy or equivalent qualification or experience.
* Designated Prescribing Practitioner (DPP)
* Member of the Royal Pharmaceutical Society (RPS)
* Membership of specialist body e.g. UKCPA
Experience
* Specialist knowledge of medicines management systems and processes in an integrated NHS
* Specialist knowledge of own field of practice gained through diploma level training and experience
* Relevant clinical experience and knowledge
* Demonstrate specialist knowledge in more than one field preferably in primary care
* Practice Research Experience
* Training and education of pharmacy staff and other Health Care Professionals
* Experience of managing change
* Experience of service improvement
Language Skills
* Welsh Speaker (Level 1)

* Flexible approach to needs of the service to undertake on call, weekend and evening working.
