Job overview

The primary aim of the therapy service is to ensure patients can live well and as independently as possible in their own homes following acute hospitalisation. This will be achieved through:

1. Proactive screening and early assessment of patient’s needs.
2. Implement therapy-specific programmes and engagement in MDT initiatives to minimise sedentary behaviour during patients’ hospital stay.
3. Support the prevention and effective management of delirium and other cognitive spectrum disorders through physical and cognitively stimulating activities.
4. Identify ongoing rehabilitation goals and provision of aids to facilitate independence and safe onward care at the point of discharge in collaboration with Oxfordshire and other areas discharge to assess services.
5. Engaging and supporting the healthy and active ageing theme of the department’s research and development agenda through the Oxford Allied Health Professions Research and Innovation Unit (AHPRU).
Main duties of the job
6. To be responsible for and manage a delegated clinical caseload including review of cases as necessary.
7. To provide holistic interventions for patients without direct supervision, working to treatment plans, protocols and accountable at all times to a registered practitioner.
8. To support patient centred rehabilitation in order to facilitate timely, safe discharge, reflecting individual choice and maximising individual’s independence.
9. To achieve and maintain competencies through skills and knowledge development.
10. To collect and record patient information in EPR in line with departmental guidelines.
11. To perform non-clinical and administrative duties within the department, as required.
12. To contribute to setting up and be responsible for maintaining operational systems within the department.
13. To contribute to the induction and training of new staff and students
14. To work flexibly across the team, covering a variety of clinical areas as needed
15. To maintain rehab equipment in the department, order, clean, communicate with suppliers
