Overview
Join Telford & Wrekin Council’s Children with Disabilities Service, which supports children and young people from 0 to 17 years old with complex health, medical and learning difficulties, and disabilities. The service is a multidisciplinary team that works to provide a holistic network of support to achieve positive outcomes and enable children and young people to reach their full potential.

Responsibilities
The social worker will:
* Plan and review social care needs for children and young people with Special Educational Needs and Disabilities, including those with complex health needs.
* Work directly with children, young people and families to ensure their views are heard and they are fully engaged in the support planning process.
* Collaborate in multi‑agency working with health, education and social care partners.
* Adhere to statutory duties including Children in Need, Children Cared for by the Local Authority, and Child Protection.
* Provide quality supervision, participate in reflective consultations, training and development opportunities, and contribute to proactive child‑focused support planning.
