We are looking to recruit a Senior Practitioner focusing on improving Continuity of Care for prison leavers and supporting the Nottinghamshire Criminal Justice Team to increase engagement with service users produced at court. Salary: £31,529,41 £32,412,09 About the role: Work closely with the CGL prison in-reach workers, Prison Healthcare, Prison partners, Probation and External Support Agencies to ensure effective coordination of Release Planning Design holistic resettlement plans to support the continuity of healthcare once our clients are released from prison Work closely with the Courts in Nottingham City and Mansfield to ensure the timely provision of DRR and ATR assessments for people deemed suitable for assessment by Probation Providing advocacy for access to partnership services Ensuring quality of service delivery through data analysis and engaging staff in service quality improvement planning. Where necessary, create team and individual performance plans to improve quality and performance. Work with your team to ensure that the practices within the team/service enable identification of service users, children and families who are at risk. Ensure that staff are competent in the delivery of interventions that reduce risk to service users. Support staff to improve service user outcomes through audits and observed practice, providing timely values-based feedback. About you: Have a strong understanding of substance misuse issues and experience of working within a similar field ideally within prison/criminal justice Be an excellent communicator, both verbal and written with good IT skills Proactive and able to work on own initiative with excellent time management and prioritising skills Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders Ability to manage change successfully in a way that prioritises the needs of service users Understand the importance of information governance processes and commit to follow and apply all necessary safeguards Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others.
