We are recruiting for a Patient Flow Co‑ordinator who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients. The primary focus of the role is to ensure flow is created in the trust, to allow patients to be placed in the right bed at the right time, supporting patient flow and facilitating timely safe discharges. This role aims to support the Trust: Ensuring support is given to wards to unblock delays and issues that may impact on timely discharges. Problem solving issues that are causing discharge delays and supporting the implementation of care when patients are discharged home. You will be working closing with Pharmacy, Site office and Patient Transport to co‑ordinate discharges from the acute trust. Escalating delays to seniors. Any other task that needs to be performed within the parameters of the role to assist with the continued discharge of patients and supporting Transfer of care Team. Main duties of the job
Manage the discharge today list and ensure nerve centre system is updated. Participate in transferring patients to the Discharge Lounge, as early as possible, and throughout the day. Manage and support the home today list on Nerve centre and ensuring patients are discharged as planned. Communicate and close working with support services, Transport, pharmacy, discharge lounge, Transfer of care Team, Site Office and Ward staff as required. Ensure that the next day discharges are shared with all support services on Nerve centre application and in real time.
