The community nursing team across Peterborough are currently offering opportunities for full time and part time hours working within CPFT.

We provide a service 7 days a week, working closely with other community services and PCN’s to provide high standard holistic care to the population of Peterborough.

You will have opportunity to develop yourself, with extensive learning and development opportunities.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.

As a community staff nurse, you will be working as an autonomous practitioner within the multi-disciplinary community nursing team. You will provide high standard of nursing care and interventions within the patient’s own home, with support from the wider team.

The patient will be at the centre of care being provided, ensuring patient and carer participation in decision making. You will be accountable for assessing, interpreting, planning, implementing, and evaluating treatment to patients within professional guidelines. Patient needs will be identified through holistic nursing.

You will have opportunity to provide support and education to peers, new staff and students.

• To work as an autonomous practitioner within the integrated community nursing team.
• To be accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines, with the support of the multidisciplinary team
• To keep the patient at the centre of care, ensuing patient and carer participation in decision making
• Provide support and education to peers, new staff non-registered staff and students
• To provide a high standard of care
• To provide a high standard of nursing intervention within a patients own home this includes lone working, with access to Specialist and Advanced Practitioners when required
• Will be expected to work as part of a 24/7 service
• To identify patient needs, agree goads and to provide appropriate holistic nursing interventions, to refer to other services and provide or order equipment
