Role Overview
Home First Response Team provides timely, person‑centred interventions that support patients to maximise their health, wellbeing and independence through a coordinated, multidisciplinary approach.
Duration, Location and Working Pattern
* Permanent full‑time post (36 hours) working 5 days over 7 days.
* Based at Royal Alexandra Hospital.
* Roles will be assessed for sponsorship eligibility in line with Home Office requirements.
Key Responsibilities
Within the Home First Response Team, you will utilise highly developed specialist knowledge to deliver a high‑quality specialist service. You will provide expert clinical intervention, education and support to patients and their carers.
Key responsibilities include:
* Supporting safe discharge of patients from hospital and preventing inappropriate readmissions.
* Delivering specialist direct clinical care, including comprehensive assessment, treatment and management of patients referred to the service.
* Working collaboratively with health, education and social care colleagues to ensure coordinated effective care for adults with complex conditions.
* Providing supervision, guidance and support to less experienced practitioners, support workers and students, and advising multidisciplinary team and partner agencies on professional interventions.
* Working autonomously to manage a delegated caseload, including assessment, intervention planning, implementation and evaluation of outcomes.
Qualifications and Experience
* Diploma or degree leading to Physiotherapy HCPC registration.
* HCPC registration.
* Relevant NHS experience.
* Documented evidence of commitment to CPD and leading in‑service training.
* Specialist interest in frailty.
