Overview
JustOT are looking to recruit a band 6 Community Care Coordinator Occupational Therapist for a locum position with our client in London.
The pay for this position is £22-£26 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.
Job Description
Provide specialist nursing care for individuals with learning disabilities and mental health needs through clinical assessment and a stepped-care model within a MDT.
Act as care coordinator, manage access to services, maintain accurate records, and ensure safe, effective practice in line with NMC standards.
Collaborate with service users, families, carers, and professionals to deliver person-centred, socially inclusive care for those with complex or challenging needs.
Responsibilities
* Provide specialist nursing care and care coordination within a MDT, applying a stepped-care model.
* Manage access to services, maintain accurate records, and ensure safe, effective practice in line with NMC standards.
* Collaborate with service users, families, carers, and professionals to deliver person-centred, socially inclusive care for those with complex or challenging needs.
Qualifications
* Registered, RMN, RNLD or HCPC
* Degree-level education
* Experience working within a community mental health setting
* Post-qualification experience working with adults with learning disabilities and mental health needs, or strong evidence of transferable skills
* Strong clinical knowledge in supporting clients with complex needs and challenging behaviours
* Skilled in risk assessment and care planning
* Confidence participating in MDT meetings and contributing to clinical discussions
* Successful candidates must hold a degree in occupational therapy and be HCPC registered
Desirable
* Experience working in partnership with families and carers
* Car owner
* EPJS experience
