Supported Assistant Practitioner/Assistant Practitioner 8998 | Maternity cover contract | £28,598 to £33,699 per annum (Scale F/G) | 37 hours per week | Norfolk & Norwich University.
We have a full‑time opportunity in our Home First Hub, an integrated team working closely with Norfolk & Community Care (NCH&C) and NHS colleagues to ensure safe and timely hospital discharge for those in community and acute hospitals across the Central Area of Norfolk. This is a new, developing team committed to supporting people to return home, access reablement opportunities and plan their ongoing care and support needs once discharged.
Role and Programme
As a Supported Assistant Practitioner (Scale F) you would join our 9‑month, bespoke support programme, enabling you to progress to an Assistant Practitioner role (Scale G) subject to satisfactory programme completion.
Key Responsibilities
* Attend hospitals and complete face‑to‑face assessments.
* Work remotely and visit the office base at least three times a week.
* Support safe hospital discharge and plan ongoing care needs.
* Use professional curiosity around information exchange and community resources.
* Work independently or as part of a team in a responsive fast‑paced environment.
Qualifications and Skills
* Experience in the Health or Social Care sector.
* Good standard of education with excellent IT and organisational skills.
* Positive can‑do attitude and ability to work at pace.
* Good listening, analytical skills and flexibility to new ways of working.
* Committed to supporting people to leave hospital safely.
