We are looking for a physiotherapist who is already working at Band 6, or an experienced Band 5 Physiotherapist, to join our friendly and supportive Community Therapy Team (CTT).

The post holder will be working in the Community Therapy Team based in SW Surrey covering Guildford and Waverley areas. We provide Physiotherapy and Occupational Therapy to adults in their community, including those who have neurological conditions, frailty or have had orthopaedic surgery. Our main base is at Milford Hospital, with a satellite office at Guildford. The role involves providing rehabilitation to patients in their own homes and providing support to staff as required.

The post holder will be supported to further develop their knowledge and skills in their chosen area of speciality. They will have professional responsibility for themselves and for the staff they supervise. They will be expected to contribute to the on-going development of clinical practice, standards, research, as well as policies, protocols, guidelines and service development.


Main duties of the job

The Senior Physiotherapist will assess and treat referred patients using evidence based practice and clinical experience, within various rotational clinical areas of community units/hospitals and patient homes.This could be a rotational or static post within a Community Therapy Team (CTT).

To assess and treat patient's in various clinical areas, including clinical units and patient homes. To support the team in the organisation and provision of an efficient, high quality and co‑ordinated physiotherapy service for patients on relevant clinical area. To undertake a defined clinical caseload, as directed by team lead. To ensure that patient and service standards and priorities are met. To provide clinical supervision to assigned junior physiotherapists, technical instructors, physiotherapy assistants and students. To participate, and teach where competent, in training sessions such as on‑call training, team in‑service and Trust mandatory training. Be an active part of the weekend service. Requirement to be available across 7 days and flexible working hours as service need requires.

Job responsibilities

Be skilled, and have relevant Physiotherapy experience in assessment, diagnosis and treatment of patients, using a wide range of clinical reasoning skills. To build on their wide base of junior experience and to expand clinical reasoning skills and continue to develop their clinical skills further in relevant areas. Formulate and deliver individual treatment programmes based on evidence based practice treatment options. Formulate discharge plans for each patient including the completion of relevant holistic assessment forms. Transfer patients to other members of the multi‑disciplinary teams for on‑going treatment within inpatient, outpatient or domiciliary settings, as they deem appropriate. Complete the relevant discharge forms/summaries. Ensure that patients give informed consent prior to treatment and, for patients who lack capacity to consent, the correct legal framework is followed, liaising with team lead where needed. Evaluate patients progress, reassess and modify treatment programmes if required. If a patient fails to progress or deteriorates to discuss with team lead. Use discharge to assess principles for D/C as soon as safe to do so, ensuring any forward referrals are completed in a timely fashion.

Participate in the induction of new starters. Participate in the appraisal process and conduct appraisals and objective setting for juniors, TIs, PTAs, once all relevant training completed. Liaise with the team lead and clinical service manager to ensure that objectives set in this process support the teams annual plan. Recognise and monitor any performance issues of staff and report to the team lead and clinical service manager. Take their guidance on the appropriate management. Provide leadership/ line management for junior PTs, TIs, and PTAs or other relevant members of the team. Be a role model for the team.

Undertake the clinical supervision of undergraduate physiotherapy students whilst on their placements. Ensure students are provided with the relevant experience required by the universities to meet the placement standards. Countersign all student assessment and treatment documentation. Discuss with the team leader the students final evaluation and assessment, and complete all assessment documentation. Liaise with the clinical service manager and university tutor re. Any difficulties in the student placement. Attend University training days to ensure up to date teaching for students.

Provide clinical supervision to junior physiotherapy staff, TIs, PTAs or other MDT staff within relevant area. This includes the planning of caseloads/ workloads. Delegate non‑clinical tasks such as checking equipment, administration tasks etc. to team and ensure they are performed to the required standard. Deputise for the team lead in their absence, ensuring smooth running of the team and adherence to safe staffing levels. Report issues or gain support directly from the Clinical service manager at this time.

Consolidate skills learnt at junior level and work on achieving the additional clinical competencies and skills required. Develop current knowledge of Evidence Based Practice and assist the team leader to integrate into the standards and policies as appropriate. Attend and contribute to team in service training as determined by their team leader. Attend all department general in service training. Complete relevant equipment competencies. Support the clinical service manager/Team Leader by providing appropriate team training, supervision and competency assessment as requested, plus signing off for competence achieved for juniors, TIs, PTAs and provide feedback to the team leader.

Requirement to be available across 7 days and flexible working hours as service need requires.

Identify stock and equipment needs to the team leader. Ensure correct prescription of aids to patients. Be responsible for equipment loans/deposits up to £50.

Provide feedback to Team Leader if any problems are encountered. Attend/ lead multi‑disciplinary team meetings. Communicate with MDT members forming both internal and external links. Ensuring that patient and staff motivation is achieved and monitored. Liaise with the referrer re any inappropriate referrals and communicate this with Team Leader. Communicate effectively, sensitively and appropriately, with patients (and Carers) who have a variety of medical, psychological and social needs including those with impaired cognitive function or reduced verbal communication.

Following appropriate training, book and document patient appointments electronically, using appropriate computer system EMIS/ Cerner. Be computer literate in order to complete objectives such as the presentation of audit data etc.

Comply with teams safe/ minimum staffing protocol. Undertake appropriate clinical risk assessments in line with trust and service policy as part of the overall risk management strategy. Complete adverse incident forms (DATIX) as per Trust policy, informing their supervisor of the incident. Comply with all Trust and department policies. Attend all mandatory Trust training and maintain up to date department records.

HCPC membership must be kept up to date and competence to practice maintained. Adhere to the CSPs professional code of conduct. Adhere to confidentiality guidelines at all times, as per RSFT Confidentiality Code of Conduct.
