Role and Responsibilities
You will be part of a vibrant community nursing team, delivering care to patients in their own environments. You will work under the guidance of the Nursing Team Leader, apply your clinical expertise to make vital decisions that impact patient outcomes, and contribute to safeguarding quality and safety. Your role includes addressing diverse health needs, including end-of-life care and patients with long-term or complex conditions. You will work within an MDT approach with the wider integrated neighbourhood team to meet the wellbeing needs of the population.
Your days will combine hands-on patient care with mentoring and professional oversight to ensure safe and high-quality care.
Requirements / Qualifications
* Relevant CSP Qualification and required community nursing experience as set out in the job description.
* Full driving license and access to a car.
Service Details
The Community Nursing Service is a 7 day service with variable shift patterns available between 8am–10pm.
