Job Summary

We are recruiting HIU Leads to support the launch of a High Intensity Use Service within South Lincs Rural PCN. The post is a 12‑month fixed‑term contract, funded for five years, with two positions: one serving South Lincs Middle and one shared between South Lincs Rural and Four Counties PCN.


Key Responsibilities

* Work face‑to‑face with high‑intensity use (HIU) clients to understand their needs and build trusting relationships.
* Coordinate wellbeing and connect clients to health, housing, social care and community support services.
* Advocate for clients, guiding them through the complex journey and promoting appropriate use of scheduled and unscheduled care.
* Facilitate interdisciplinary discussions, advise colleagues on best practice, and oversee the delivery of the service.
* Lead efforts to remove barriers and reduce stigma associated with HIU, promoting equality, diversity and safeguarding across the service.
* Provide professional expertise to the Lincolnshire Integrated Commissioning Board – including report writing and presentation of evaluation reports.
* Maintain effective record‑keeping, ensuring compliance with GDPR and demonstrating outcomes.
* Contribute as a member of the Neighbourhood network, supporting vulnerable populations and addressing population health management and health inequalities.


Person Specification

Qualifications / Education

* GCSE level or equivalent in Maths and English
* Personalised Care Institute accredited training for Social Prescribing
* Personalised Care Institute accredited health coaching training
* Personalised Care Institute e‑learning modules (PCSP, shared decision‑making or equivalent)

Experience

* Motivational interviewing and coaching for health and wellbeing
* Supporting vulnerable adults in a person‑centred way
* Experience in the voluntary and community sector
* Case‑load management and working effectively in teams and collaborative settings
* Provision of social prescribing interventions
* Cooperation with health professionals, voluntary sector providers and a wide range of stakeholders
* Use of Microsoft 365 and other collaborative online tools
* Knowledge of saving and data protection legislation (GDPR)
* Awareness of equality, diversity and safeguarding policies
* Flexibility to work outside core office hours and to travel across the PCN locality
* Disclosure and Barring Service check – required for the role

Skills

* Excellent communication and interpersonal skills
* Strong safeguarding awareness and up‑to‑date knowledge of legislation
* Proactive, self‑motivated and able to work independently and in teams
* Ability to prioritize, manage time effectively and work in a busy environment
* Proficiency in SystmOne and other professional documentation tools
* Flexibility to work from home where tasks permit
* Capacity to adapt to change and cope with uncertainty
