Job overview

Detailed job description and main responsibilities
Discharge planning
* Manage a caseload of patients, coordinating with social services and community personnel for timely transfers of care.
* Assess and set discharge dates, complete necessary documentation, and liaise with care providers and families.
* Order and coordinate equipment and support timely discharges.
* Lead on developing discharge plans within 24 hours of admission and ensure accurate documentation in EPIC.
* Maintain relationships with social services, CCGs, and care homes to facilitate transfers.
Clinical Triage
* Ensure high-quality transfer notes and participate in system-wide transformation work.
Education and Training
* Deliver training, lead educational programmes, and mentor staff and students.
Research and Development
* Stay updated with research, lead audits, and contribute to improvements in discharge processes.
Person specification
Registration
* Registration with NMC, HCPC, or Social Work England is essential.
Qualifications
* Degree or diploma in health or social care-related fields, with relevant experience.
Experience
* Extensive practice experience with complex patient discharges.
Knowledge
* Advanced understanding of hospital discharge systems and procedures.
