The role
Agency Social Worker Post – Taf Ely Community Mental Health Team, Rhondda Cynon Taf CBC We are looking for an agency social worker for a nine-month period to join the Taf Ely Community Mental health team. The team covers the Taf area of Rhondda Cynon Taf, roughly from Llantrisant to Pontypridd to Tafs Well. The work will be focused on individuals with mental health needs. The team consists of a social work team manager, principal social workers, social workers (including AMHPs), community psychiatric nurses (CPN), nurse team lead, psychiatrists, psychologists, and an occupational therapist. The expectation is that the role is site based 5 days a week, Monday to Friday. A 4-day week can be considered. There is scope to agile work from home, dependent on the needs of the service. We do not allow definite allocated days working from home because the team needs someone who is able to respond to the requirements of the team, attend the office, cover the duty desk, and undertake visits and face to face assessments. Being able to travel independently in the locality is essential. Duties Work as part of a team and understand the strengths and challenges of multi-disciplinary working and be able to deliver an effective and efficient service to the public. Safeguard and promote the wellbeing of adults through the provision of high-quality social work practice in assessment, care planning, and review. Co-produce and implement care and treatment plans and act as a care co-ordinator in accordance with key legislative requirements of the Mental Health Measures Wales (2010), Work within and understand of the principles of the Social Services and Well-being (Wales) Act (2014). Undertake mental capacity assessments and work within the principles of the Mental Capacity Act 2005. Be familiar with WARRN risk assessments and assess risk in a collaborative and structured way that balances risk with autonomy and choice. Identify and promote individual strengths and independence. Work in collaboration with other agencies and adopt a multi-disciplinary approach where appropriate. Ensure that electronically held records, including case recordings, assessment, care and treatment plans, and reviews are completed and updated in accordance with departmental policy and procedures. The role will be for a social worker covering maternity in the Taf Ely community mental health team based in Dewi Sant health park, Albert road, Pontypridd CF37 1lB
