What you will do:
* Carry out the Occupational Therapy process alongside service users and carers, encouraging choice and fostering independence.
* Conduct need assessments and evaluate risks collaboratively with service users and carers.
* Perform risk assessments regarding moving and handling tasks, ensuring a safe environment for everyone involved.
* Guide service users and carers in identifying and purchasing aids and adaptations while supporting adherence to the Children’s & Families Act 1989 and 2004, and the Care Act 2014.
* Foster effective joint working by coordinating with professionals from the statutory, voluntary, and private sectors, such as Social Care Staff, Health Care Professionals, and Housing Teams.
* Lead and contribute to multi-disciplinary assessments and case conferences as needed.
* Utilize various tools and options available to support service users in crafting their support plans using a person-centred approach that promotes their personal goals.
