This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
This position serves as the admission coordinator for the Mental Health Residential Rehabilitation Treatment Programs (MHRRTP) of the Finger Lakes Health Care System. The position is responsible for the initial screening assessments with Veterans and for evaluation of admission requests to the MHRRTP. The incumbent ensures the applicants meet program requirements based upon the applicant's identified needs/goals. The incumbent also manages the MHRRTP waitlist, acts as a liaison to referring clinicians, and coordinates the admissions process. In coordinating the admission process, the incumbent serves as a point of contact with programmatic responsibilities for admissions related issues, for incorporating VISN and national policies and guidelines into local policies and practices and for developing productive working relationships with referral sources such as community agencies and other VA facilities.
The Social Worker assists the participants' families and clinical member in coordinating care in the community and resource referrals during MHRRTP stay, when clinically indicated, and works in unison with the interdisciplinary teams in promoting a patient centered culture that assures that quality health care services are provided to the Veterans promptly, safely, courteously, and compassionately and that their significant others and/or major stakeholders are appropriately involved. The incumbent provides patient/family/caregiver education and support, and actively promotes patient self‑management and personal health responsibility.

Pay
Competitive salary and regular salary increases.
Paid Time Off
