The Blaenau Gwent Community Resource Team is seeking an exceptional Assistant Practitioner to join our dynamic integrated team. The Community Resource Team is a collaborative service between Aneurin Bevan University Health Board and Blaenau Gwent Social Services, dedicated to providing timely interventions from the right professionals. As an Assistant Practitioner, you will help support adults in remaining in their own homes, working within a multidisciplinary team including Physiotherapists, Occupational Therapists, Support Workers, Social Workers, Medics, and Nurses. You will also collaborate with services such as the Rapid Response Team, Community Hospitals, and Care Agencies.
The team's role is to enable support in home settings, prevent hospital admissions, facilitate timely discharges, and assess long-term care needs. The post requires identifying service users needing preventative or therapeutic interventions and managing caseloads independently under supervision of HCPC-registered professionals. You will carry out treatment programmes, using problem-solving, time management, and assessment skills, respecting service users' privacy, dignity, and diversity.
We deliver high-quality health and social care services for adults in Blaenau Gwent. The service operates 7 days a week from 7:30 am to 8 pm, 365 days a year. Staff work across these days, including weekends and bank holidays on a rota basis.
Desirable qualifications include the ability to speak Welsh; however, English and/or Welsh speakers are equally encouraged to apply. A valid full driving license and access to a car during contracted hours are essential.
