Job Title:Children’s Social Worker – Disabilities Team
Location:Worcestershire
Rate:£33.30 per hour
Are you passionate about making a difference in the lives of young people with disabilities? Worcestershire’s Young Adults Disabilities team is looking for a dedicated Children’s Social Worker to join our close-knit team. This is a fantastic opportunity to work with young people aged 16-25, primarily those with Learning Disabilities, but also those with physical disabilities, neuro-diverse needs, or complex health needs, as they transition from Children’s Services into adulthood.
Responsibilities:
You will work in a multi-agency manner with Worcestershire Children First (WCF), Adult Services, Special Educational Needs Department (SEND), health colleagues, schools, and colleges to ensure young people receive the full range of options they deserve. Your role will involve using Liquid logic in both adult and children’s systems and working effectively within children and adults legislation. You will be responsible for working with Children and Families to achieve the desired outcome, discharging care orders, attending and participating in Strategy meetings, conducting Looked After Children Reviews, completing Care Plans, Child Permanence Reports (CPR), completing Pathway Plans, completing Children and Family Assessments and working with Multi-agency partners.

Qualifications and Experiences:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK license
* Post-qualified experience
* Degree level or equivalent in Social Work
